Body Masculinization Cost
On this page
Insurers treat body contouring as cosmetic, including for people whose top surgery the same insurer funded, so plan on paying for all of it yourself. Prices swing with the surgeon's reputation, the city, and how many areas go into one session, and the way liposuction is quoted makes two prices hard to compare unless you know what to ask.
Cost by procedure
| Procedure | Typical US self-pay | Typical Bangkok self-pay | Notes |
|---|---|---|---|
| Liposuction, one area | $3,000–$5,000 | Priced per area | Flanks, abdomen, thighs, arms, chest sides |
| Liposuction, a typical session | $4,000–$15,000 | $2,000–$6,000 | Two or three areas together; cheaper per area than done separately |
| Hip reduction | $5,000–$12,000 | $2,500–$5,500 | Liposuction of hips, outer thighs and flanks; the bone can't be changed |
| Pec implants | $8,000–$15,000 | $4,000–$7,500 | Custom implants add a few thousand; implants sometimes billed separately |
| Abdominal etching | $5,000–$12,000 | $2,500–$6,000 | Abdomen alone at the low end, abdomen plus flanks at the top |
| Fat grafting to shoulders or pecs | Roughly $5,000–$15,000 | Usually quoted with the liposuction | Almost always done in the same session as liposuction |
| Skin removal or body lift | $10,000–$25,000+ | Varies widely | For loose skin after major weight loss; a separate, larger operation |
How liposuction is quoted
Liposuction is priced by area, and there is no standard definition of an area. One surgeon's "abdomen" is another's "upper abdomen" plus "lower abdomen", and "hips" can mean the hip alone or the hips, outer thighs and flanks together. Two quotes that look $3,000 apart can be for the same operation described differently. Ask each surgeon to itemise the quote by named area and to mark them on you or on a diagram.
Then ask whether the figure is all-in. Surgeon, anaesthetist and facility are often separate lines, and a quote for the surgeon's fee alone looks far cheaper than the bill will be.
What's usually included
- Surgeon's fee
- Facility and anaesthesia (smaller areas are sometimes done under local with sedation, which costs less)
- Standard follow-up visits
- Usually one compression garment
In Bangkok, hospitals quote a package that typically includes a night in hospital and the first garment. The packages I have looked at over the years are clear about what is inside. What sits outside is usually extra nights, a second garment, medication to take home, and any touch-up after you have flown home.
What's often extra
- Consultation fee
- Blood tests and any clearance needed before the date
- The implants themselves, for pec implants
- A second compression garment, which you will want so one can be washed
- Lymphatic drainage massage, which many surgeons recommend after liposuction. In the US that is roughly $80 to $200 a session, and courses of five to ten sessions are common.
- Touch-ups. Minor irregularities after liposuction are common enough that touch-ups are a normal part of the conversation, and practices differ on whether the first one is free.
- An overnight stay for large-volume liposuction
- Time off work: one to two weeks for desk work after any of these, longer for physical jobs and longest after pec implants
Combining with other surgery
Liposuction of the sides of the chest is commonly added to top surgery for something in the region of $1,000 to $3,000. That is far less than the same work as its own operation, because you are not paying for a second anaesthetic and a second facility fee. Flank or hip liposuction can sometimes be added to a hysterectomy, though not every surgeon will do it. Genital surgery is usually kept separate, because a compression garment and a fresh groin do not mix. Pec implants are rarely done with top surgery, since the surgeon wants to see the healed chest first. See combined procedures.
Insurance
Not covered as gender-affirming care in practice. Some plans will pay for skin removal after massive weight loss where there is documented recurring skin infection or functional problems, but that is a separate route with its own criteria and has nothing to do with gender. See insurance.
UK
Private only; the NHS gender pathway does not fund body contouring. Liposuction in the UK typically runs £3,000 to £8,000 for one or two areas. Pec implants and abdominal etching are offered by a small number of surgeons, so get more than one quote and ask how many they do a year. See NHS.
Canada
Private only. Pricing is broadly similar to the US in Canadian dollars, with liposuction roughly CA$4,000 to CA$10,000 a session depending on areas. See Canada.
Australia
Private only. Liposuction runs roughly AU$4,000 to AU$12,000 depending on areas, and pec implants AU$10,000 to AU$18,000. Australia tightened its cosmetic surgery rules in 2023, including who may call themselves a surgeon and a requirement for a GP referral first. Check the surgeon's registration on the public AHPRA register rather than trusting the clinic's website. See Australia.
Abroad
Turkey, Mexico, Colombia, Thailand and Poland all draw people for body contouring, generally at a fraction of US prices. The Bangkok column above is the one I can vouch for. Liposuction abroad is a volume business, and results vary more than for almost anything else on this site, so the surgeon matters far more than the country.
Budget for the trip as well as the operation. Most surgeons want you nearby for about two weeks, and flights plus two weeks' accommodation can come close to the price of a single-area procedure.
The flight home needs planning in its own right, because a long-haul flight soon after large-volume liposuction stacks two clot risks. That combination is what I push back on hardest when people plan these trips. Ask how much volume the surgeon plans to take and how long they want you on the ground before you fly. See surgery abroad.
Before you spend the money
Testosterone does a fair amount of this on its own, given two or three years. Fat moves off the hips and thighs, the waist straightens, and training fills out the chest and back. Most surgeons will tell you to give it that long and reach a stable weight first, because until then they are aiming at a moving target. Fat removed does not come back, but the fat left behind still grows with weight.
Surgery earns its cost for fat at the hips and outer thighs that neither T nor training has shifted, and for a chest that is flat after top surgery but stays small however hard you train. Pec implants and etching suit people who are already lean; on a softer body the implant reads as a lump and the etched lines fill in. And liposuction only takes fat out. If the skin is already loose, removing the fat leaves it looser, so ask the surgeon directly whether you would need skin removal too.
Reducing the cost
- Wait until you are a couple of years on T and at a stable weight. You may need less, or nothing.
- Combine areas in one session, and add chest-side liposuction to top surgery rather than booking it later.
- Ask whether a small area can be done under local anaesthetic.
- Get the touch-up policy in writing before you pay.
- Financing is available; see financing.
