Body Masculinization Surgery

Body masculinization is the set of procedures that reshape the torso and hips toward a male silhouette. Testosterone does most of this on its own over a few years, moving fat away from the hips and thighs and toward the abdomen, and training does the rest for many people. Surgery is for what those two leave behind, which is usually fat that will not move and a hip-to-waist ratio that bone dictates.

It is the smallest group on this site and the most cosmetic in how it is treated. The procedures are the same ones cosmetic surgeons do for anyone, liposuction, implants and contouring, applied with a masculinising aim. What makes them gender-affirming is the goal, not the technique, and that shows in how they are funded.

The honest framing is that surgery here is a finishing step. Testosterone first, then training, then, if the shape still reads wrong, the procedures below.

Who body masculinization is for

Anyone whose body shape still reads as feminine after testosterone has redistributed their fat, which takes two to three years, and who has done what training can do. The common complaints are hips that stay wide, a waist that stays narrow relative to them, fat at the thighs and lower abdomen that testosterone did not move, and a chest that is flat but not built.

Surgeons will want you settled on testosterone and at a stable weight, because liposuction on a body that is still changing is guesswork. Nicotine has to stop. A letter of support is sometimes asked for, mostly for funding rather than for the surgery itself.

The people I have seen it suit least are those hoping surgery will substitute for testosterone or training. It shapes what is there; it does not build it, and the consultations that go badly are the ones where that was not understood beforehand.

Before you book

Give it a year of testosterone and six months of training you actually did before you decide whether surgery is needed. Most people who come to this group early would not have needed it later, and the ones who still do arrive knowing exactly which feature they want changed.

Choosing between them

Liposuction is the workhorse and the one most people have, because fat distribution is the main thing testosterone leaves unfinished. Hip reduction is liposuction aimed at the one area that bothers people most, with its own limits, because bone sets the floor. Pec implants are for a chest that will not build, usually after top surgery has flattened it. Abdominal etching is the most cosmetic of the four and the most dependent on already being lean.

Pick by feature. If the hips are the problem, hip reduction. If it is general softness, liposuction. If you are lean and flat-chested and want definition, implants or etching.

At the consultation

Ask what the result will look like if you gain or lose five kilos afterwards. Liposuction moves fat; it does not stop you making more. A surgeon who answers that honestly is planning for the body you will have, not the one on the table.

The body masculinization procedures

The four procedures below share a purpose and little else. Liposuction and hip reduction remove fat; pec implants and abdominal etching add or reveal shape. They are usually done alone, occasionally paired, and all of them are done by cosmetic surgeons rather than gender specialists, which makes choosing the surgeon a matter of looking at their male body work rather than their trans work.

Liposuction and Body Contouring

Liposuction of the hips, outer thighs, flanks and lower abdomen through a few small incisions, to take the curve out of the waist. Permanent for the fat removed, but what is left behind moves with weight and hormones, so timing decides the result.

Best for: someone settled on testosterone at a stable weight, with curves at the hips or waist that are fat rather than bone

Hip Reduction

Hip reduction is liposuction aimed at one thing, the fat over the pelvis and outer thighs, to straighten the line from ribs to thigh. It narrows the silhouette; it cannot narrow the bone, which sets the floor on what the result can be.

Best for: someone whose hips are wide from fat over the pelvis rather than the pelvis itself

Pectoral Implants

Solid silicone implants placed under the pectoralis major, after top surgery, to give a flat chest the bulk and lower edge that training could not. Permanent and shaped like a muscle, but they do not flex, and they can shift or show on the wrong chest.

Best for: lean people a year or more past top surgery whose chest stays flat however hard they train

Abdominal Etching

Liposuction shaped to leave the grooves between the abdominal muscles visible, usually done with VASER. It reveals muscle already there under a thin layer of fat and does nothing for anyone carrying real abdominal weight. The result lasts only as long as the weight does.

Best for: someone already lean and stable in weight who can see a hint of definition when they flex

Body masculinization recovery

Liposuction recovery is a compression garment for four to six weeks, bruising that looks worse than it feels for the first two, and a final shape that takes three to six months to appear as the swelling goes. Desk work in a week. Implant recovery is closer to top surgery, with restrictions on lifting for six weeks. Etching is liposuction with more bruising.

None of these is a hard recovery. What they share is the long wait for the result, and the temptation to judge it early.

Body masculinization cost

Body procedures are the least likely on this site to be covered anywhere, because they are cosmetic procedures in every classification system regardless of the goal. Self-pay everywhere, with prices that follow the cosmetic surgery market in each country. Bangkok is a common destination for liposuction and implants, and the price gap is large. Where it fits

Body work comes last, after testosterone has redistributed fat and after top surgery, if you are having it, has set the chest. Pec implants in particular need a healed top surgery to work with. None of it depends on or affects genital surgery, and it can be combined with a hysterectomy or top surgery under one anaesthetic if the surgeon is willing. See surgery order.

Eligibility

Won't testosterone change my body shape anyway?
