Top Surgery Cost
On this page
The price of top surgery moves more with the surgeon, the city and the paperwork than with the technique. The ranges below are the self-pay bands quotes have fallen into over the last couple of years. They are a starting point for reading a quote, not a substitute for one. The number that varies most is not the headline price but what it covers, and what the second operation costs if you need one.
Self-pay ranges by technique
US figures are all-in packages (surgeon, anaesthetist, facility) where the practice quotes that way. Bangkok figures are hospital packages for international patients, before flights and accommodation.
| Technique | US self-pay | Bangkok package | Notes |
|---|---|---|---|
| Double incision | $7,000–$13,000 | $3,500–$7,000 | Most US quotes land at $8,000–$12,000 |
| Peri-areolar | $7,000–$13,000 | $3,500–$7,000 | Usually priced like double incision; liposuction is part of it |
| Keyhole | $6,500–$12,000 | $3,500–$6,500 | Shortest operation; often the lower end of a surgeon's pricing |
| Inverted-T | $8,000–$14,000 | $4,000–$7,500 | Longest operation; ask whether an overnight stay is included |
| Minor revision | $0–$5,000 | $500–$3,000 | Often free or facility-only within the first year; a conversion to double incision is priced as a new operation |
Two surgeons in the same city can be several thousand dollars apart for the same technique. Big-city surgeons with long waiting lists sit at the top of each band, and hospitals cost more than surgical centres. Price does not track results in either direction.
What a quote should list
Ask for it itemised, in writing. A complete one has:
- Surgeon's fee
- Anaesthetist's fee
- Facility or hospital fee, and whether an overnight stay is in it
- Pathology on the removed tissue
- Follow-up visits, and for how long
- The first compression vest
- The revision policy, stated as a price
What usually sits outside it: the consultation ($100–$400 in the US, sometimes credited against surgery), pre-op bloodwork, prescriptions, extra vests, scar tape and silicone, travel, and time off work. Hidden costs puts figures on all of those. Pathology and a separate facility bill are the two I see arrive as surprises most often, because a surgeon's office can quote its own fee in good faith and still not know what the hospital will charge.
Comparing two quotes
A worked example with made-up but typical numbers. Surgeon A quotes $9,500 all-in, including pathology and one minor revision within a year at no charge. Surgeon B quotes $8,000, which turns out to be the surgeon's fee alone. Add a facility fee of $1,800, anaesthesia at $900 and pathology at $300, and B is $11,000. A minor revision with B is another $1,500 to $3,000 in facility and anaesthesia costs, even if the surgeon waives their fee.
Whether that revision is likely depends on your chest more than your technique. Across techniques, surgeons' own figures put touch-ups at one in ten to one in four people; see revision. The case where the policy matters most is a borderline small-scar candidate, because if peri-areolar or keyhole leaves loose skin, the fix is usually a double incision, and most practices price that as a full operation. If you are in that position, ask what a conversion would cost you before you choose. Which technique covers how to tell if you are borderline.
US insurance
Many US plans cover top surgery as medically necessary, usually with one letter of support and prior authorisation. What you pay is your deductible and coinsurance up to the plan's out-of-pocket maximum, which for most people having surgery means a bill in the low thousands and sometimes the full maximum.
Three things change that number. The liposuction and chest contouring lines on the claim are the ones insurers most often refuse as cosmetic, so ask the office which codes it will submit. Many well-known surgeons are out of network, and out-of-network reimbursement often covers only part of the fee. If no in-network surgeon nearby does the operation, you can ask the insurer for a network gap exception. And deductibles reset with the plan year, so if you have already paid towards this year's, or expect other big bills next year, the date you book changes what you pay. Insurance covers prior authorisation, denials and appeals.
UK
The NHS funds top surgery through the gender clinic pathway, but as of 2026 the wait from GP referral to surgery is measured in years. The NHS page has the pathway and what the waits have been running at. Private top surgery in the UK runs roughly £6,000 to £11,000, and London sits at the top of that. Most UK private quotes bundle the hospital and anaesthetist; check whether pre-op tests and follow-up are in.
Canada and Australia
Every Canadian province funds top surgery, with its own approval route and waits from months to a couple of years. Self-pay is possible to shorten the wait; clinics that publish prices have quoted roughly CA$8,000 to CA$12,000. Provincial plans often exclude contouring and revisions without a functional problem. See Canada.
In Australia, Medicare rebates cover a fraction of the surgeon's fee, and the private hospital bills separately. With private hospital cover the out-of-pocket gap is roughly AU$5,000 to AU$12,000; without it, roughly AU$10,000 to AU$20,000. See Australia.
Thailand
This is the market I know from the inside. A Bangkok package usually already includes the hospital, the anaesthetist and one or two nights on the ward, which a US quote often does not. Nobody funds it for an international patient, so it is self-pay by definition.
The package price is not the trip price. Surgeons want you nearby for two to three weeks before you fly, so add flights, accommodation for that stretch, food and a companion if you bring one. For someone flying from the US or UK, the saving is real but usually a good deal smaller than the two surgical numbers suggest. A revision a year later means another trip, which is why the people I have seen do well budget for that possibility up front. Surgery abroad works through the full sum.
Europe and Mexico
Poland has several experienced top surgeons at roughly €3,000 to €6,000. In Germany, the Netherlands, France, Spain and Italy, private quotes have tended to fall around €5,000 to €9,000; see Europe. Mexico is used mainly by Americans at a substantial discount to US prices, with more variation between clinics than you find in Bangkok. The trade is the same as Thailand's. Follow-up is at a distance, and a complication that shows up at week three happens after you have flown home.
Bringing the number down
- Ask whether the surgeon operates at a surgical centre as well as a hospital, and what the difference in price is.
- In the US, an HSA or FSA can pay with pre-tax money.
- Surgeon payment plans, medical credit and grants all exist, each with catches. Financing goes through them.
- A mid-range surgeon who is in your insurance network will often cost you far less than a famous one who is not.
Get three itemised quotes, ask each surgeon what you would pay for a minor revision and for a conversion, and compare the totals rather than the headline figures.
