  5. Why the ranges are wide
  6. Using these numbers

These are the self-pay prices for every FTM procedure on this site, in one place. The US and Thailand figures are the same ranges as the Costs table on each procedure page. Everything is in US dollars, rounded, and dated by the review line at the foot of the page. They are what surgeons and hospitals publish or quote at consultation, not what an insurer pays and not a quote to you. If you have coverage, your out-of-pocket cost is a separate question; see Insurance Coverage, Appeals and Denials.

Thai hospitals price in baht, so the dollar figures move with the exchange rate. The US and Thailand ranges are not like for like either. A Thai figure is usually a hospital package with the nights in hospital included, and it leaves out the flights and the weeks you spend in Bangkok afterwards. Surgery Abroad does that arithmetic.

I coordinate patients into Bangkok hospitals, which is a direct interest in one of these columns. Treat the Thailand figures as the ones to check hardest rather than the ones to trust most,, and read them against what the same operation costs where you live before they change anything. UK prices are in a section further down.

Top surgery

Procedure US self-pay Thailand self-pay Notes
Keyhole $6,500–$12,000 $3,500–$6,500 Often the lower end of a surgeon's top surgery pricing
Peri-areolar $7,000–$13,000 $3,500–$7,000 Similar to double incision; ask what a revision would cost
Double incision $7,000–$13,000 $3,500–$7,000 Most US quotes $8,000–$12,000
Inverted-T $8,000–$14,000 $4,000–$7,500 Usually the top of a surgeon's range
Revision $0–$5,000 minor $500–$3,000 minor A conversion to another technique can cost full price

Top Surgery Cost covers what moves a top surgery quote within these bands.

Bottom surgery

Procedure US self-pay Thailand self-pay Notes
Hysterectomy $10,000–$20,000 $4,000–$8,000 Hospital pricing at the top, surgical centres lower

Urethral lengthening as a separate stage or repair $10,000–$25,000 $4,000–$10,000 Included in the stage price when done with the phalloplasty
Scrotoplasty $5,000–$15,000 alone $2,000–$5,000 alone Almost always inside the meta or phallo quote

Erectile implant $15,000–$35,000 plus the device $8,000–$20,000 Its own stage, usually a year or more after phalloplasty

Across all flaps, the phalloplasty page gives $50,000 to $150,000 or more in the US (some full pathways are quoted above $200,000 with hospital fees) and $25,000 to $60,000 in Thailand, where the top of that range is a package that bundles hysterectomy and vaginectomy with stage one. Bottom Surgery Cost explains how the stages are billed.

Face and body

Procedure US self-pay Thailand self-pay Notes
Forehead and brow $8,000–$20,000 $3,500–$8,000 Custom implants add scanning and manufacture
Jaw and chin $6,000–$20,000 $3,000–$9,000 Chin implant alone at the bottom

Hairline and beard $6,000–$15,000 $2,500–$7,000 Priced per graft
Liposuction $4,000–$15,000 $2,000–$6,000 Roughly $3,000–$5,000 per area in the US
Hip reduction $5,000–$12,000 $2,500–$5,500 Less if added to top surgery
Pec implants $8,000–$15,000 $4,000–$7,500 Implants sometimes quoted separately
Abdominal etching $5,000–$12,000 $2,500–$6,000 Abdomen plus flanks at the top

Facial procedures are usually booked two or three at a time, and a combined quote is normally less than the sum of the rows because you pay for one anaesthetic and one theatre booking.

The UK and other countries

The tables carry no UK column because every figure in them comes from the Costs table on the procedure page it links to, and for most of these operations there is no UK equivalent to draw on: the private market is thin outside chest surgery and hysterectomy, so a UK column would be numbers with nothing behind them. What follows is only what UK providers and hospitals publish themselves, in pounds, at the time of review.

Chest surgery. London clinics that publish a price list start peri-areolar and keyhole work at roughly £7,500 to £9,000, double incision with free nipple grafts at roughly £9,000 to £10,500, and inverted-T at the top of that. Adding liposuction to the chest takes published starting prices past £12,000. Across UK providers as a whole, private top surgery runs roughly £6,000 to £11,000, with London at the top; Top Surgery Cost has more.

Hysterectomy. Not priced as gender surgery in the UK. You are buying a general gynaecological operation at private-hospital rates, and published self-pay guide prices run roughly £5,800 to £11,200 depending on the surgical route and the region, London at the top. The first consultation and the pre-op tests are billed separately, typically £200 to £350 for the appointment.

Face and body. Priced as ordinary cosmetic surgery. Published UK self-pay ranges are roughly £4,400 to £9,800 for rhinoplasty and £2,000 to £8,600 for liposuction. The clinics that list facial masculinisation as a single line start it around £8,600.

Metoidioplasty, phalloplasty and the stages around them. Not publicly priced. No UK provider publishes a self-pay figure for these, and the numbers that circulate come from comparison sites rather than from anyone's own price list, so this page leaves the cell empty rather than fill it with something you cannot trace. In practice people either wait for the NHS route or self-fund abroad.

Two limits on all of the above. A published starting price is a floor, not a band, and the thin end of the UK market means one or two price lists can carry a whole procedure. For the public routes, see NHS Pathway and Waiting Lists, Coverage in Canada, Coverage in Australia and Coverage in Europe.

Why the ranges are wide

Facility fees. In the US the surgeon's fee can be a third or less of the bill. The same surgeon operating in a hospital rather than a standalone surgical centre can produce quotes thousands apart, which is why hysterectomy has such a wide US range.

What the quote covers. Some quotes are all-in (surgeon, anaesthetist, facility, post-op visits, garments), and some are the surgeon's fee alone, with the rest arriving as separate bills. That is the most common reason a number moves after someone thought it was settled. In the consultations I sit in on, I ask for anaesthesia, pathology and follow-up to be named on the quote line by line, because those are the three that turn up later as separate invoices.

Staging. Phalloplasty and full metoidioplasty are staged, and a quote may be for one stage or the whole sequence without saying which. A $40,000 phalloplasty quote is often stage one only. Ask for the whole plan priced in writing.

Geography and demand. Big coastal US cities cost more than the Midwest or South, and London more than the rest of the UK. The most sought-after surgeons price at the top and carry the longest waits.

Using these numbers

Use them to sanity-check a quote, not to shop by price. A US top surgery quote of $4,000 is a reason to ask what's missing, and so is one of $25,000. In a decade of coordinating patients through Bangkok hospitals I haven't seen price predict the result. Within the normal band, the surgeon's healed-result photos on bodies like yours, and how they talk about the cases that went wrong, tell you far more.

The surgical fee is also not the whole budget. Consultations, hair removal, travel, time off and revisions come on top; Hidden Costs of Surgery puts numbers on them. If the total is more than you have, Financing: Loans, Grants and Crowdfunding covers the options, and Private vs Public Surgery weighs paying now against waiting for a funded route.
